The Special Assignment Nurse (SAN) is a unique member of the interdisciplinary team. The SAN moves between nursing roles and departments to provide support and coverage as needed. SAN primary responsibilities include: vacation relief in patient care and admissions, the admission of patients to hospice care, case management and special projects as assigned. In all situations, the SAN is a member of the core interdisciplinary team responsible for the care and case management of patient/families admitted to hospice service.
