Staff Nurse

Staff Nurse needed for the Family Care Center (50%). As a team leader, will assess and provide interventions based on needs of adult and pediatric populations in a busy primary care setting, as well as provide patient education, staff telephone triage and evaluate effectiveness of care provided. Requires BSN or Diploma or AD in Nursing, previous experience with chronic disease management, managing both pediatric and adult populations and ability to rotate through Saturday morning clinics and late nurse position on week days. Knowledge of the electronic medical record, strong telephone triage skills, experience with office procedures and background in diabetes management, hypertension, hyperlipidemia and other chronic diseases desired. Position is 50%.
