HealthPlus of Michigan has an excellent opportunity for an individual to join our leading health insurance organization. The Disease Management Coordinator is responsible for designing disease management program(s), implementing activities, monitoring and measuring outcomes of programs for chronic conditions.
The successful candidate will have a current, valid Michigan license to practice as a Registered Nurse without any conditions, limitations or restrictions, and a Bachelor’s degree in a clinical or health related field or equivalent experience (which equates to a nursing degree, plus three years of professional nursing experience). Must also have three years of chronic disease or population-based disease management experience in a clinical, public health or managed care environment, as well as, experience in program development, basic statistical analysis, and clinical outcome documentation.
