Oncology Case Manager – Atlantic City, NJ
We are currently looking for someone who shares our passion for helping others! We have a new and exciting opportunity for an Oncology Case Manager to provide outpatient case management assistance to oncologists and attending physicians to identify patients earlier in the disease process. Our goal is to ensure that the members of our Fund receive timely, appropriate and medically necessary care consistent with high quality efficient cancer care and the most appropriate use of clinical oncology services. This nurse will work also to ensure that members participate in shared decision making for cancer treatment and are directed to the appropriate provider and level of care. The Oncology Case Manager may also utilize non-traditional resources to improve care for individual patients and work with other internal staff such as patient advocates and clinical pharmacist to utilize appropriate community-based services and end of life care to achieve goals of care and cost savings for case managed patients.
