CNR Health Care Network, part of Beth Abraham Family of Health Services is a voluntary, community-based healthcare organization delivering a full range of continuing care services to elderly, disabled and chronically ill New Yorkers.
We have the following opportunities available:
• Assistant Directors of Clinical Care - Responsible for management, assignment and training of staff. One position for our LTC AIDS Program (with Bi-lingual Spanish preferred); the other for our LTC Program in Brooklyn.
Each requires RN plus BSN and 2 years’ home care experience as a Community Health Nurse in a CHHA or LTHHC Program, or 4+ years of home care experience and 6 credit hours of Public Health and Principles Management training.
• RN Intake Coordinators/Screeners - To foster relationships and identify potential reference sources as well as pre-screening candidates for our Center for Nursing & Rehabilitation programs in the Bronx (Bi-lingual Spanish required), Brooklyn and Queens.
Each requires a Bachelor’s in Nursing or health-related field and 2+ years each of Med/Surg and LTHHC or CHHA experience (with intake experience preferred), plus basic computer skills, exceptional networking ability, scheduling flexibility, current NYS driver’s license and car access. PRI certification and second language a plus.
• FFS Field RN Home Care - To provide direct hands-on nursing care and health teaching in patients’ home environment. Requires NY RN licensure, CPR and reliable transportation. 1+ years of Community Health experience and BSN preferred.

• Director of Clinical Services (Brooklyn) - To oversee budgeting, staffing, LTC regulations, MDS process and reimbursement. Requires NY RN licensure and 3+ years as a Director of Nursing Services with proven track record in achieving excellent clinical outcomes.
