NURSE CARE MANAGER #123121

This is a fantastic opportunity for a BSN with five years of exp., incl. two years of community health to be part of the future of healthcare in our community and the nation. Managed care organizations utilize preemptive nursing interventions to achieve the goals of improving care while reducing system costs, through such activities as case managing high risk populations and monitoring groups of patients within managed care settings for support interventions.

These nurses work within a private practice setting, collaborating with physicians and midlevels who deliver primary care services. As case managers, nurses may perform work telephonically, within the practice setting or in the patient's home in order to assure the most comprehensive care management delivery by all providers, collaborating with the patient's specialist providers in order to assure maximum delivery of services at the lowest cost.

Bilingual is a plus! In this role you will integrate community services and care delivered in private practice settings. You will assist patients to avoid unnecessary hospitalizations, readmissions and unnecessary ED admissions. You will be transforming care delivery while integrating practice based quality-improvement. Extensive training is provided.
