Responsible for managing regional deployment, integration, and enhancements for the Patient Centered Care (PC2) transformation program. Primary duties may include, but are not limited to:
-Manages PC2 internal integration with care management and alignment with all partners such as Medicare and Medicaid.
-Oversees PC2 transformation market specific needs.
-Establishes and maintains PC2 management policies, procedures, and protocols. -Manages PC2 operations and processes.
-Provides guidance regarding the market specific strategy, and relationship building and intervention opportunities within the practices/clinical review process. Identifies care management opportunities and progress toward program adoption.
-Oversees the development of action plans to support provider care management transformation.
-Partners with case/disease management departments to facilitate clinical processes between the company and provider offices.
-Serves as a liaison with state/region provider contracting account managers.
Requires a BA/BS in Nursing; 5 years of experience in care management; 2 years of prior management experience; or any combination of education and experience, which would provide an equivalent background. Current unrestricted RN license in applicable state(s) required. Travel required (approx. 25%)
