Catholic Hospice, Inc. has been serving terminally ill patients in Dade and Broward counties for over 20 years. We provide excellence and compassion in Hospice Care to people of all faiths. Our offices are located throughout Dade and Broward Counties and we recently opened an In Patient Unit in Hialeah Gardens.

GENERAL DESCRIPTION:
The Hospice Registered Nurse is responsible for evaluation referrals and coordinating patient/family needs for admission to the Hospice program. Under physician's orders, the RN is responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the terminally ill patient/family unit. The RN utilizes assessment, intervention and teaching skills in caring for the patient/family unit.
QUALIFICATIONS:
1. Commitment to Hospice philosophy and mission.
2. Current Florida License as a Registered Nurse.
3. One-year clinical nursing experience required. Hospice nursing experience preferred.
4. Ability to communicate effectively orally and in writing.
5. Computer literate, or willing to learn
6. Willingness to travel to any location in Dade County and or Broward County.

1. Must be able to lift and move patients according to accepted nursing standards of body mechanics.
2. Must possess a current Florida driver's license.
3. Must be able to provide own transportation to various locations in Catholic Hospice’s service areas as required by duties.
4. Must be able to lift disposable patient supplies for transportation and delivery to patient's homes up to 25 pounds.
5. Must be able to sit, stand, bend, and squat.
6. Must be able to produce legible, handwritten patient care notes.
7. Must be able to hear and speak for telephone/general communication.
DUTIES:
1. Evaluates assigned referrals for appropriateness for admission and verifies terminal diagnosis and prognosis.
2. Contacts patient/family within twenty-four hours to set up appointment to explain hospice services.
3. Communicates to patient's attending physician, referral source, and Catholic Hospice office staff patient's/family's decision regarding hospice services.
4. Performs the patient assessment and physical on admission to determine the current health status of the patient and initiates the written plan of care with the IDT. When applicable, discusses and initiates the written plan of care with the appropriate Nursing Home/A.C.L.F. staff.
5. Communicates with appropriate Nursing Home/A.C.L.F. staff on an on-going basis and attends Nursing Home care plan meeting when appropriate.
6. Educates team members regarding nursing aspects of patient/family care.
7. Teaches caregivers how to properly care for patient/family care.
8. Provides on-going assessments of physical, psychological, social and spiritual needs of the patient/family unit and initiates appropriate intervention or referral.
9. Serves as coordinator of care of the patient and family.
10. Administers medications and treatments prescribed by the attending/hospice physician. In the nursing home setting, monitors prescribed medications, treatments and reports effectiveness to the Nursing Home Team Manager.
11. Reports all changes in medication and treatments to the Patient Care Manager.
12. Provides accurate and appropriate documentation of patient/family services.
13. Actively and cooperatively participates in the care planning process, updating care plans according to the needs of the patient.
14. Reviews Hospice section of Nursing Home/A.C.L.F. charts for appropriate forms, i.e., Patient/Family Informed Consents, DNR, IPOC, etc.
15. Assists with providing bereavement counseling to the family and/or primary care giver as determined by the Interdisciplinary Team.
16. Evaluates services provided by the Nursing Assistant every two weeks with periodic on-site supervisory visits.
17. Attends Interdisciplinary Team Meetings to report on patient status.
18. Participates in the team responsibility of providing patient care twenty-four hours a day, seven days a week.
19. Provides progress reports to the attending physician about changes in patient's condition or plan of care according to hospice policy.
20. Recognizes, reinforces and enhances the role of the volunteer in providing Hospice services.
21. Represents Catholic Hospice at community nursing functions.
22. Participates in hospice activities such as Memorial Services, Community Education, etc., as requested by the Patient Care Manager.
23. Demonstrates adherence to Catholic Hospice policies and procedures and State and Federal Rules and Regulations.
24. Adheres to a professional business attire dress code.
