Relief is 16 hours/week, including weekend and holiday rotation.
Part-time is 24 hours/week.
* Perform assessment, planning, implementation, facilitation and evaluation of discharge plans for patients from newborn to adults requiring intermittent in-home skilled service
* Perform as member of interdisciplinary team based on specified group of attending physicians
* Work collaboratively with the hospital's health care team and act as a patient/family liaison with hospital services, managed care providers and community health providers
* Perform the hospital utilization function to ensure appropriate use of resources and compliance with insurance contracts
Requirements:
* Knowledge of home health and third-party criteria for reimbursement, discharge planning and utilization review experience
* Current PA RN licensure
* Bachelor of Science degree in Nursing
* Excellent communication skills
* At least one to two years med/surg and recent acute hospital case manager and discharge planning experience
