DEFINITION
The Branch Manager is a registered professional nurse who is responsible for supervision, coordination, and scheduling of clinical services. The Branch Manager ensures that care plans are written and followed according to physician orders and agency standards. The Branch Manager also reviews all written documentation and coordinates decisions regarding changes in levels of care with their clinical staff. The Branch Manager reports directly to the VP of Clinical Operations or regional Director of Clinical Operations. The Branch Manager may assist other office staff or clinical staff as needed.
QUALIFICATIONS
Essential Qualifications
• A Registered Nurse with current licensure to practice professional nursing in the State.
• Graduate of NLN accredited school of nursing with a BSN degree is preferred.
• Minimum of three (3) years experience as a professional nurse. Must have some experience in Medicare home health nursing, and some supervisory or management experience.
• Must be able to perform wound care and IV functions, as well as other general nursing functions.
• Basic computer skills. Able to use MS Windows based software, MS Office packages, email, and internet.

• Is self directed with the ability to work with little supervision.
• Is flexible and cooperative in fulfilling all obligations. Able to accept constructive criticism and use it in a positive manner.
• Has good interpersonal skills with understanding of supervisory management concepts.
RESPONSIBILITIES AND DUTIES
Essential Functions
• Directly supervises branch staff. Oversees Team Manager supervision of field staff. Maintains updated and current patient information.
• Conducts conferences with each clinician reviewing:
o patient status/condition changes;
o potential patient problems;
o upcoming recerts, discharges, transfers;
o status of paperwork, MD orders.
• Reviews weekly clinician activity and productivity of full-time clinicians. Ensures that quotas are being met and schedules are current and submitted per timetables.
• Reviews patient documentation for timeliness, accuracy and legibility: (including)
o time sheets;
o progress notes, care plans, medication profiles;
o discharge/transfer summaries;
o post hospital assessment/MD order forms.
• Reviews SOC paperwork for new patients. Ensures compliance, accuracy, appropriateness of services ordered.
• Coordinates the initial services and screens referrals for appropriate assignments. Identifies potential risks and coordinates treatment planning for unassigned patients.
• Assists team members in establishing therapeutic outcomes, setting priorities, and developing appropriate plans of care.
• Reviews and evaluates each patient case through a variety of means such as home visits, record review, and patient care conferences. Ensures patient record is accurate and up- to- date.
• Instructs and guides team members to promote more effective performance and delivery of home care services.
• Is available to clinicians during operating hours to assist clinicians as appropriate. Acts as liaison to physicians and community resources as needed.
• Orients new staff that are hired into the branch, utilizing the agency’s established orientation protocols, and in a timely manner.
• Prepares and conducts performance evaluations of team members and establishes a performance improvement plan as appropriate: (including)
o home peer review visits;
o annual evaluations;
o interim evaluations as appropriate for performance level.
• Supervises LVN team members and their patients monthly and PRN through home visits. Reviews/updates care plans as needed.
• Assists in the planning, implementation and evaluation of the clinical department ongoing and annually.
• Participates in the quarterly record review audit and communicates findings and recommendations pertaining to team members.
• Maintains good working knowledge of all rules, regulations, accreditation standards, and agency policies regarding program and staffing requirements, quality of care and standards of clinical performance. Ensures compliance with State and Federal Law at all times.
• Participates in activities to ensure ongoing accreditation status.
• Assists in the investigation and resolution of patient/community related complaints and reports findings to the Vice President of Clinical Programs.
Other Functions
• Functions as a team member to solve problems within the clinical department as well as other departments to improve overall function of the agency.
• Monitors hospitalized patient status and pending discharge home care needs to ensure continuity of care, reviews plan for consensus and plan modification if needed. Notifies clinical staff as indicated.
• Assures that the agency is prepared to meet the patient’s needs at the time of hospital discharge. This may include making arrangements for special medical supplies or appliances, training agency personnel regarding unfamiliar procedures or problems pertaining to the patient’s care, and communicating information regarding the patient to agency personnel. Identifies potential risks and coordinates treatment planning.
• Serves as an educational resource to the hospital, SNF, or physician staff concerning home health services, such as conducting training and serving as a consultant to the hospital or SNF for establishing home care policies and practices necessary for patient care.
• Provides back-up coverage to other team managers, Intake, Admit, and patient visits when needed.
• Maintains staff necessary to achieve highest clinical standards and to ensure coverage of projected patient visits. Makes hiring decisions, ensures appropriate training and field supervision as needed.
• Participates in the on-call rotation or ensures coordination of clinical activities after hours with other agency departments and appropriate personnel.
• Attends staff, clinical manager meetings, inservices , and community activities as requested.
• Serves as Branch program advisor to the Executive Director, Board, and Advisory Board. Reports pertinent information related to program activity as requested.
• .Is respectful of individual’s rights in interacting with patients/families and co-workers.
• Accepts responsibility for errors and follows reporting protocols (Incident Report).
• Monitors patient and referral source evaluations and concerns on a continuing basis,and intervenes immediately as indicated. Keeps VP Clinical Programs informed of any problems as they occur and makes recommendations for resolving them.
